Tangs sometimes react weird with ich, but that still looks bacterial to me. It’s possible the fish got injured and the area got infected.

Not sure it does look like a bacterial infection to me. How big is your tank? When did you get your PBT? We might need a little more info to properly diagnose him .

As @exnisstech suggested, some more pictures, video and fish history will be helpful.

I see what appears to be white spots on the body may be on the pectoral fins. Can you confirm?

The scaring you see may or may not be related. How long has it been present?
