Pinched Mantle & Open Siphon?

Hi! I've had this Derasa clam for about a month now and I've notice the profession of a possibly pinched mantle on one side of the clam. It is about 1.5 to 2 inches long and retracts when shade is provided/scared. Additionally my bubble tip Anenome had decided to creep over and sting the mess out of my clam to the point where the threads have detached and I was able to move him into a comfier spot. Free of anenome invasions or shading from light. Although, I've noticed that the siphon has been wide open for a couple days, nearing a week now and I worry that my clam is not happy.
I asked my LFS and they told me to feed with pyto- so I did this morning.

Parameters are 1.025 for salt, 0 for nitrates, 0 for phos, 0 for ammonia

I added a picture of when I first noticed a partially retracted mantle to how it is now

It's really hard to tell from those pics, can you get a clear shot from above and close up from the front? Turn your lights on the whitest setting possible when taking pics.

Also, can you tell us more about the system/tank? Age, how many fish, how much you're feeding? In a zero nutrient tank phyto may help with providing some nitrogen, but I wouldn't count on it as the main source of food, that should be light. PMD will progress along the mantle, you'll also notice the mantle will have "crinkle" appearance as well.

Update:

Thank you guys for replying- I had taken more pictures to update this post a few days ago but I had also decided that even if my clam did not have a pinched mantle that it would only benefit from a FWD... That being said I dipped it for 25 mins and the results are not well. 100% sure that it is gaping now and i plan to remove it from my system if I do not see any progress in the next few days. It's a shame, but I can only learn from instances like this I suppose... attached are a few pictures I took with your guys' comments in mind. I have not gotten a picture of the clam recently as it is closed for most of the day but when it is open I can see that the siphon is a lot more open than in the pictures attached and one side of the clam is completely retracted while open. (I'll add pictures when I am able to capture it)

Update:

Sorry to hear about the clam :( I don't see a lot of shell growth. After a month it should have something there. 0 nitrates and phosphates are not good. How about the light situation? What kind of light and par?

Sorry about the clam, but agree that it hasn't been growing for quite some time. Have you ordered James Fatherree's book yet? If not, that would be my next purchase.
