Hammer skeleton exposed?? Is it dying?

I noticed my hammer looking like this yesterday. His skeleton was sort of exposed and it was very concerning. When I shower my LFS employee, he told me the coral was probably going to die. When I got home today, the coral looked completely normal so I’m just a bit perplexed. Is it dying? What can I do if so?

It may be losing the head. I would check your water and see what's up. Phoshates and Nitrates being main concerns.
I checked everything yesterday but literally nothing is concerning/unstable. I have another hammer and a torch that are doing great as well. What are the chances of recovery and is there anything I can do?

seeing some color within base, its likely a little too much flow, low calcium, and even elevated phosphate.
