As others have said, you have some issues in that tank and should really rehome the Bristletooth ASAP.
I'd vote disease as the most likely culprit so far from what you've posted.

Indeed! I tried a Bristletooth Tang in my 40GB, he was good for a few months, and although they don't get as big as other tangs, they get a decent size.Bristle tooth tang is way to big for that tank... They need at least 100G...
Or…. He has a chromis and a tang….That is a chromis not a tang. Did you think the store sold you a tang?
