I thinned out the dead skeleton from underneath using a dremel and bone cutters. What I found out are worms.
Interesting. Just reviving this thread to try to understand this better.

Can you explain how you "thinned out" from "underneath"?

Did you turn it over and cut in from the back? At what angle?

Sorry, I'm just trying to understand better.

And what color were the worms?

Check to see if your clowns aren't irritating it. Sometimes clowns can get obsessive and physically contact a coral to the point of irritation. In addition, gobies might be inadvertently getting sand on them.
I would agree that no phosphates is part of the issue. If you're not having luck feeding, try checking them after lights out later in the night for tentacle extension. Perhaps they have not "learned" to eat during daylight hours

Have you increased your lighting at all recently?

I'm having somewhat of a similiar issue. My acans are still growing, but they don't plump up as much as they did in the past. Meanwhile, my other coral are growing at record pace right now. It's a bit of a mystery.

Thus far my 2 theories are metals in my water. My alumnium and vanadium have been creeping up. And also I'm now wondering if bumping up my lights from 80% to 100% is the cause.

I never feed them directly, so I guess that could be the case. But here is a picture of them after 1 year in a tank, and 6 months ago. This is what I'd like to get back to.

They still plump up a little like I said, but not like this.

Acans don’t like much light. Keep the off center and to the side. Also don’t like much flow. Feed them Mysis weekly. Get your phosphate up a bit by dosing some Neo Phos. Undetectable is not good for any coral
