Yellow tang not doing so good after freshwater dip

By the title my yellow tang is not doing good after a freshwater dip. he's laying sideways on the sand in the qt tank , i left him in the freshwater for 4 minutes. i made sure it was the same temp as the tank and same ph. it seems like he's really struggling should i put him back in my reef tank? i'm really scared he'll die.. i did this to check if he had flukes.

I take it you didn't see any flukes? Did the tang have any symptoms prior to the dip? Where is the fish now, since it isn't in your reef tank? I don't see any downside to putting it back into the reef with the lights out and see if it recovers.

FW dips are not totally risk-free, but it is unusual for a fish to die from them unless there were serious underlying conditions.

I saw a few little white circles in the bucket when i did the dip, before the dip he wasn't eating acting very shy and hiding for about a week. it's in my quarantine tank. not sure if my corals would be ok with lights out. he's still breathing and now laying against the glass and not on the sand.

I've also had him a month prior to this, he was acting all good and eating with no signs and then this suddenly happend.

Seems like he's a goner.. breathing has slowed down and is still laying on the sand, i don't think he'll make it.

Slow breathing is better than rapid breathing. Add oxygen if you can and assure you have proper temp and salinity
Temp range 76-79
Salinity 1.024-1.025
How is ammonia level?

Went to test the levels but by that time it was to late, i feel horrible..

Lights out would not have hurt your corals...they can go days without light.
