This is bacterial and may have stemmed from skin irritation and has progressed. As a medication that is easily absorbed by a fish, treat with Seachem Kanaplex in a separate tank and with added aeration using an air stone. It will take a couple of days to start taking effect based on severity. Fish also looks slightly thin. If eating, what foods are you offering?

@vetteguy53081 thank you, I have kanaplex and can try that. Can I use it in the QT tank with the royal gramma? Just these two are present.

Tomini is not eating. Tried Nori, OGO, Mysis, also Chromaboost is available as the royal gramma is eating that.

Try live or frozen brine shrimp as well as LRS Fish or herbivore frenzy
Focus on bacterial issue though which will lead to diet resuming

Based on the fish having been in copper power (assuming for at least 3 days), plus the low respiration rate, I think you can rule out any protozoan issues. I agree w/ @vetteguy53081 that this looks like a bacterial infection (it being one-sided also point to that)

The not eating, and the large size of the lesion means this is very serious. Antibiotics take 3 to 5 days to work, but the fish may not have that long to go, so try to get the treatment started ASAP.

@Jay Hemdal thank you for the guidance, the lesion spread suddenly and rapidly and the fish has already died and is removed. Is there any concern for the royal gramma that is still in this QT tank? The gramma seems to be fine and isn’t showing any signs of infection. It is mostly hiding but seems to just be the nature of the fish. Should I treat the tank with the kanaplex or just stay the course with the copper and monitor things from here. Thank you!

Sorry to hear - that was a very fast infection. There is some concern for the gramma because of how virulent that was. However, the bacteria that infect fish are almost always a species commonly found in aquariums in some numbers, so the individual fish needs to have some immune problem to get infected.

I guess I would have the Kanaplex on hand and then dose the tank if the gramma shows symptoms. Kanaplex isn’t risk free, so dosing it with no active disease really isn’t best.
