Tough to see, but the scales seem raised up, that indicates this is an internal issue. It could be bacterial, but I’m worried it might be Uronema. That disease isn’t very contagious to other fish, but it isn’t treatable and almost always fatal once you see a lesion this size - sorry.

He is definitely dying, I try to catch it but it moved into the rock work. It is moving very slowly, and not eating. I touched it before it moved.
Thank you. I have read it is most tanks, but should I dose anything to the tank since the royal gramma getting/having it? Also should I be more concerned about my other fish, I guess manly my clowns?The angle of the lesion indicates that this is most likely Uronema, sorry.
