My blonde naso tang has 2 white spots under one of its fins. He doesnt use that fin much it is mainly swimming with one fin. The white spots are raised bumps. I got it a few weeks ago and devoleped this a week or two after I got it. Hes still eating, not doing much grazing on the rocks but still eats nori and mysis shrimp.

Welcome to R2R! Can you please post white light photos and a video? Helps ensure correct diagnosis. Youtube links work best. Here's an article with other helpful information.

The pics are awfully dark and best under white light intensity
This may be bite marks or light injury . Fish looks slightly thin and may be in alignment with an ongoing blonde Naso issue
Not eating can also be associated with any aggression if present
Try live brine shrimp if available as well as LRS herbivore frenzy

I agree - clearer photos and/or video are needed to really try to diagnose this - and even then it may be tough to tell.

Not using the fin is a sign of possible injury though.
