My anemone was looking great for the first month but now seems to be taking a turn.

It’s been moving around the tank for the last 3-4 days and this morning the mouth was gaping significantly more than the picture below.

I’d rather nip this now then when it gets even worse, what does everyone think?

I don't think it is infected, but I think it does not like the condition where it is at. Does it have enough light and current? Is the water condition pristine?

it moved down to that spot last night, the other available areas where it was prior has heavy light and moderate flow. water conditions are great, everything else in the tank is doing awesome.
