Nothing I can definitely put my finger on.
I'm leaning towards too much success - the coral were getting rather crowded and I'd been meaning to do some pruning for a while.
May simply have been too much coral for a 90 gallon system.
If so, it's solved now. lol.
yes, I’ve lost at least 4 tanks
but on a positive note, if tanks didnt crash or have negative issues this hobby and supporting industry would eventually die out

I dosed nitrates when my tank didn't need it. I didn't crash my tank but I had a bad algae outbreak.
BTW this isn’t a bad thread per se but more benefit to me would be a list of tanks over 10yrs old that didn’t crash
I was only using an Apex to run a heater. The Apex froze and held the heater on. I was not using a secondary controller. Lost a lot of coral and livestock. I had the tank running for four years. I ended up shutting the tank down and got out of the hobby for a few years. I am back now and have redundancy with my heater controller. I am not hating the Apex either as this was 10 years ago when it crashed. My take away is always have two controllers on a heater so you do not over heat.

I had one major heatwave melt down in 2002 soon after a move/upgrade. Happened during a vacation, air conditioner was in the back of the truck.
Fast forward to 2012, newly rebuilt system doing well for a couple of years until lack of maintenance caused all my LPS to die, many of the softies did fine though.
I didn't dial this tank back into raising LPS until around 2020. I got it cleared of algae and success with LPS again for several years.
Early into 2023 I removed a large mushroom covered rock, moved some rocks into the sump and replaced the big rock with cycled dry rock. Most of the remaining corals fared well, lost some heads on one established hammer. I have been dealing with cyano and GHA for about a year or so. I'm likely due for a more regimented GHA removal and WC schedule and possibly removal of my shallow sand bed which up to this point was never a problem. Also the growth of my hammer/frammers are making it difficult to get good water flow, likely time to trim.

The tank crashes I’ve had were slow. I lost about 50 acro frags to stn in a frag tank. It took me two years to try again. I also lost a display tank full of zoas (five year growth) to an unknown cause. They just detached from the rock and blew away to wither out. In both cases I couldn’t pinpoint the cause.

I think I am going through something right now unfortunately. A cooling fan fell into the display. I think something from the fan may have leeched into the water. All of my corals are closed or dead/dying. I am doing water changes and added carbon but I am at a loss on how to overall save my baby.

I had a couple also. Probably something stupid that I done or didn't but trying not to let it happen again

As mentioned above, the best treatment is almost always large water changes and addition of activated carbon.

Exact same situation for me 15 years or so ago. Fish lived minus 1 but all corals died.
