Grouper swimming abnormally

This is my greasy grouper which I've had for about 5 months

He was fine just yesterday but it seems that there has been some problem, he does usually always hide or stay in one place but he seems to be coming out a lot more often

His diet for the past 5 months were a combination of squid, clam, anchovies, whole head and shell shrimp and pieces of mackerel and of other fish, for extra nutrition I always juice the anchovies up with multivitamin soaked pellets

I feed him about 2 times a week and sometimes only once a week for digestion

Could this issue be regarding constipation? Or could it be a swim bladder disease?

Before I got him I did make sure to quarantine him with praziquantel and metronidazole for 2 weeks to kill any internal parasites

The last time he had eaten was 3 days ago, he ate some squid and clam and some anchovies

Will need clear pics under white light intensity and even video for best assessment. Anchovies and even mackerel are oily and may or may not be a contributor. Switch for scallops

This is not a good sign and im not convinced Bladder or constipation but more injury such as impact with object or becoming moribund.

Is there anything I can do? He was fine just yesterday so what could of happened? His stomach does look a little paler and he seems to be swimming all over the tank

If eating, try Brine shrimp which acts as a laxative. Assure ammonia, salinity and temperatures has not become elevated.

I currently only have freezed dried brine shrimp, ammonia is 0
Salinity is 1.024
Temperature had raised to 30 to 31c

I had expected it to be indigestion so i increased the temperature from 29 (usual) to 31

I have other 2 eels in the tank with is a pebble tooth and a fangtooth and they seem to be doing fine

Here is a more clearer observation of the grouper

He is indeed in a place where people usually walk around and does get startled even if you go near the tank to see him, when he gets startled he darts off but he does it usually and just maybe this time it just wasn't his day, i saw him like this when I came from college

But is there any chance of saving him?
Or should I just wait and see

You can’t really treat an injury, just try to keep it calm and give it time.

IDK though, if it smacked into something, there should be some visible damage or missing scales…..

IDK though, if it smacked into something, there should be some visible damage or missing scales…..
I see some visible scratches near the gills where's it's a bit whitish
