Blenny - White blotches - bacterial?

Noticed my blenny was out and breathing heavily with white blotches, and what looks like his ribs showing (yet he is fat).

Noticed something odd a day ago when he didnt come out for the morning feed. Normally he is front and center. So whatever it is, it has hit suddenly.

The top of the dorsal fin has gone silvery white. I dont think its Ich or Velvet, spots are too big.
Bacterial maybe?

No new fish or coral have been added to the tank in a year or more. Two clowns look fine, no blotches or odd behaviour. Royal gramma also looks fine, and as fat as always.

Thoughts?

hard to see in the darker bluish light and may be flukes or even sand grains.
Please repost pics under pure white bright light

White light photos - I do see some spots. Might be Ich, or ich + something.

Since you've had the fish for more than a year and no new additions to the tank, and no other fish have symptoms, it is very unlikely to be any of the protozoan diseases like ich or velvet. Same with flukes.

That pretty much leaves a bacterial disease. However, the sudden onset is odd. Do you have a treatment tank available?

I dont have anything to treat bacterial and it is 9pm at night. Ill move him to a temporary tank overnight and head to fish store first thing tomorrow.
Not sure he will make the night though.

I noticed both sides of his mouth have white - that never used to be there, its on both sides in the same area.

You will want seachem neoplex or ruby rally pro but agree, appears as bacterial lesions or even bite marks
What other fish are in the tank?
Looks like something clowns or a royal gramma would do

Does not adding anything in a year include corals, rock, etc?

Yep - nothing. No rock, coral etc. Tank has been on cruise control while I work on a new tank.

Good news - he survived the night, but has been upside down for all of it. Still breathing, so there is a chance.
I dont think Seachem Neoplex can be sold in Australia, but i know my LFS has some other brand he imports.

Sadly he did not make it. LFS thought it was bacterial as well, but too lately identified to save him.
