I just got my Bellus Angel female few days ago. It swims vertically and looking like it's trying to jump out of the water. If not, it will just get sucked by the powerhead like a dead fish. When I try to grab it, it swims again and slowly goes back to the powerhead.

Please take a look at the video I took. I've gone ahead and setup a 10 gallon QT, but I'm not sure which treatment plan I should go with.

Oh - it is in really bad shape! That is called "piping" and it is a sign of severe stress or disease. Under blue lights, it is difficult to see more, can you report a video under white lights?

It looks like it caudal (tail) fin and the pectoral (side) fins are all eroded. That is making it difficult for the fish to swim properly, the fin erosion could be from damage that has gotten infected, or from a primary bacterial infection.

I would for sure let the store know that it is in this condition - it may have had issues when you bought it.....

Thank you

Treatment-wise, the only thing I can suggest would be a broad spectrum, gram negative antibiotic such as neomycin or kanamycin, in a treatment tank. However, I'm not very confident those medications will have enough time to work....sorry.

Sorry but this fish is thin and moribund in which I do not see this fish eating or swimming normal moving forward and A fish I would have Never purchased in this condition. A QT will offer less turbulence than shown in video but again, I see a slim chance of recovery
