Anemone caught in powerhead, concerned about mouth

I have a rainbow bubble tip and have had it for about 9 months now (or a little more). It has been happy and stayed in the same spot for all of it's life with me. I have two clownfish that are being hosted by it since May.

Today I came home and it was stuck in my MP40. The foot was not attached to it and it had one or two tentacles that had been cut off. It looks fine in general, i guess. I am worried about the mouth and the foot though.

For the past few weeks we thought it might be cloning or splitting because it had an extra head. Here is a picture of what it looked like a week ago:

Not sure what caused it to detach and go floating. Will be checking water params today or tomorrow. Last water change was the weekend before last and it was a 20-25 gallon water change on my 75+sump system. Everything seemed fine until today. I currently have my MPs turned super low and put the anemone back in its normal spot waiting for the foot to attach again. What else should I do besides wait and see and check the water?
