This anemone is very stressed and seems to be expelling its zooxanthellae which is its color and energy source. Some reasons are:
Too much or too little water light
elevated nitrate
elevated phosphates
Lack of feeding
Immature tank
Temperature too warm
High salinity
Traces of copper

Moderate light and water flow is adequate. Feed meats such as mysis shrimp or chopped krill.

I checked my parameters again , problem is phosphate , i did a 20% water change , i fed anemone and changed it location , is there any hope that coral becomes back healthy?

They tend to be forgiving and you may need to do daily water changes to reduce the phos number. If feeding the tank reef roids, may be part of issue
