Please provide a closer pic(s) under same lighting. Fish is distant in pic and may be mucus cones or cryptocaryon. What I can tell you regarding either of the two, They treat the same and fish will have to be quarantined and treated with either coppersafe or Copper Power at therapeutic treatment level 2.25-2.5 For a FULL 30 days (do not interrupt this 30 day period) monitored by a reliable Copper Test kit such as Hanna Brand- No API brand. Also monitor Ammonia levels while in quarantine with a reliable test kit and add aeration during treatment using an air stone.

The big white splotches aren't typical ich spots.

Any symptoms, like rapid breathing or flashing? Is it eating?

What are the water parameters?

No rapid breathing and I’m not sure what flashing is.

Water parameters
Salinity: 1.225
nitrate:0ppm
Nitrite:0ppm
Ammonia:0ppm
High range ph:
7.8-8.0

And yes its eating

Flashing would be quick movements like it's scratching or trying to dislodge something.

That doesn't look like ich to me. Maybe bacterial or something else causing that coloration.

Lack of O2 and stress causes hippo tangs to turn white
From the tail end it definitely has Ich. Either treat with Copper , PolyGuard, PARAGUARD, PraziPro

Make sure it still eats and make sure there’s adequate water flow and UV sterilizer to the main tank

Okay. I see some spots on the caudal fin now on my PC rather than phone.

Seeing the other thread, my diagnosis is that the clown tang has this hippo pinned in the corner. And it looks like it has a minor ich infection. Getting this one in a QT to lower its stress and let it heal, and probably doing copper treatment, would be the thing to do. Clown tangs can be really nasty by the way.
