Its been improving for a while, but still hides.

I managed to get some close shots. Could you make a diagnosis from these pictures?

could it be hexamita and the beginning of the “hole in the head”?

As said, HITH from Hexamita is a freshwater affliction. Hexamita infects marine fish, but does not cause HITH. HLLE is similar looking in marine fish, but a different cause.
The fish looks a bit thin in the belly and behind its head - can you feed it more, more food and more often?

As said, HITH from Hexamita is a freshwater affliction. Hexamita infects marine fish, but does not cause HITH. HLLE is similar looking in marine fish, but a different cause.
The fish looks a bit thin in the belly and behind its head - can you feed it more, more food and more often?

Yes, I can. The gramma is not super hungry. She eats, but not as voraciously as my clownfishes. Gives a bite to food only when passing by.

I feed once per day normally.

I’m dosing again some praziquantel (esha gdex) and hope this helps, if it has intestinal worms. In any case, seems like the fish does well for some days, then the issue appear again…

I would feed smaller amounts 3x/day. This species pecks at floating plankton all day long, so more frequent feedings would better mimic its feeding strategy in the wild.

I took some extra pictures of the belly area.
I don’t know if that white stringy area inside the belly , right under the skin, are feces, the intestine or worms.

I don’t expect further replies, but maybe the image can help a similar case here in the forum.
