Fish only, live rock. No corals.

So - you cannot then use the invertebrates as "canaries in the coal mine" in terms of water quality issues versus gill disease.

Others have suggested adding an air stone, do that right away. API test kits aren't very good, but they still should tell you of any bigger issues for the parameters you test for.
Any chance of a foreign chemical getting into the tank?

If the aeration doesn't fix it and you are pretty sure it isn't a toxin, then just mostly leaves gill disease and the most common one of those is velvet, Amyloodinium. The best treatment for that is copper, but you won't be able to dose copper in a tank with coral sand and rock.
