Is he sick ? I never seen him thin like this (we can see his fish bone) and he has weird spots on his body (its really small you maybe have to zoom on the picture) ? Is this a disease ? Can we do something about it ?
Thanks for your help.

The little holes may be remnants of ich or mucus plugs or punctures from an urchin. Any unusual swimming, scratching or hiding behavior?
What foods are you offering and is it eating?
Its beyond thin and will need fatty diet such as:
LRS Herbivore diet, plankton, Mysis shrimp, spirulina brine and Formula2 frozen.

There was a raise of phosphate in the tank (about 0.3) and there was a leak in my tank so the salinity went down a bit. Now everything is back to normal.

The fish is not hiding nor scratching and swims/feeds as usual. We used to give him only Bionautic granulat and now that we saw that it's really thin we added frozen Artemia and nori leaves to his diet.

If it's remnants of ich, is there a way it will be spreading in my tank and is there a way to prevent it ?

Thanks for your help

Welcome to Reef2Reef!

The fish is very thin, how long have you had it? Is it eating well? If so, you may need to feed it more food and more often.

I can't rule out that those spots are not marine ich - if so, they will tend to drop off and reappear in different locations on the fish every few days, and will generally increase in numbers.

We have this fish since 2 years, he is eating well and we give more food now.

We had a raise in phosphate so we reduced the food. And since this fish is slower than the others, I guess he could barely eat since the other fish were faster.

If it's ich, what do I need to do to prevent it to spread to other fishes?

Thanks

The best treatment for ich is to move all of the fish to a treatment tank and medicate them with Coppersafe or hyposalinity. You can try managing it in place, here is a post about that (but it doesn't always work):
