Help with Treatment / Diagnosis

Hey guys,

I have a weird one and need some help. I've had this Pink-tail Trigger for 6 months, While she was in QT which was well over 2 months, I noticed this lesion of some sort developing. Her tank mate was a large black trigger and they got along well with the exception of a few scuffles over food.

I'm wondering if this is an injury that has become infected, it's getting worse, first I noticed one large spot and now there's 2, her face is swollen and has been, that was the first sign I noticed.

QT Protocol was copper at therapeutic levels for over a month , 2 rounds of Prazi, and a treatment of GC and Melafix.

I was in the process of getting my 300 gallon fish only system completed and while transferring her to the main display, tried
to inspect the first spot on her mouth, thought maybe it was a tooth or she ran into a rock, it appeared like a fatty mass and would not pull off. Didn't want to stress her too much, and placed her in the main display.... (No way in hell I'll be able to catch her now)

She is eating well and acting normal. Looking for input on what this can be and how to treat, again this is a fish only system
and I don't mind treating the display. Any advice appreciated, for reference this fish is 4-5"

These appear to be lesions from some type of injury or past infection. Assure tank water quality is good and I would treat in a separate tank using Seachem Kanaplex with added aeration

So the trigger has had this for about 4 months and its gradually getting worse? It sure looks like an injury, but the slow worsening implies that its infected. Most infections either get better or much worse over 4 months time - the only one I know of that grows that slowly is Mycobacterium, fish tuberculosis. It can leave granulomas on a fish's skin like this. There is no effective treatment for this, but it is not always fatal.

Thanks @vetteguy53081 , do you see any issues with treating in the display? Catching any fish in this tank will be a complete nightmare. Again this is a Fowler tank, that has only been cycled for a week or so… I can replace any inverts in there which are minimal….

Agree - with the idea that it's likely an injury with possible secondary infection. Is the fish still eating, etc? One issue is that in a way you already treated at least with some antibiotic - and it has gotten worse (metronidazole) - of course that might not have been the correct antibiotic.

Would you be able to get a picture - and put an arrow pointing to both lesions (I see one of them. To treat your display will require a large amount of antibiotic - (it's 300 gallons, correct) - You will have to estimate the total amount of water - including the sump. You would need to remove all carbon, skimming, etc - and if you're going to do it - I would use kanamycin or neomycin (and I suppose kanamycin would be the best choice). - However, realizing that you're risking your bio filter, and your live rock, etc. Be very careful in the dosing

My recommendation would be to use a fish trap and remove the fish to a hospital tank if possible. Is there any chance that there is a foreign body lodged in part of the mouth? (i.e. something you might be able to remove if the fish was out of the water)? - I'm also not recommending surgery - but only if you could easily reach it with something like a sterilized needle, etc . since it seems so external. (I would not do this unless you're really comfortable trying). Another comment - it looks like you have a very nice assortment of fish etc - do you have anyone local (LFS, etc) - that could look directly at it and give you some advice - either on catching the fish - or other treatment options besides treating 300 gallons?
