I have a young tank and recently introduced 2 Ocellaris Clownfish and a bicolor Blenny.
The Blenny appears to have some loose skin, almost like a flimsy bubble or what a blister would look like on a human, on his head, but it’s not easy to see with the naked eye until you get close and he is still. Even more difficult to get a photo.
I found this old thread with a description that fits exactly, and a solution mentioned.

I wanted to know if anyone could confirm this as the issue most likely and if there are any newer, more favorable remedies. This thread is 12 years old.
