I had the unfortunate timing of noticing ich on my Threadfin and Longnose Butterfly about a couple weeks out from an extended vacation I was about to take. Entirely my fault, as all fish were bought quarantined so I suspect it was brought in through a piece of macro that I deemed safe. Anyway after weighing my options and with the short time frame that I had I made the choice to put my tank into hypo in the hopes that it would solve the problem. Salinity was checked by a Hanna reader while I was gone, and three weeks later upon my return I was dismayed to see that there were still signs of ich on a few of my fish. At that point I was all ready to go nuclear with copper in my DT as I had no holding tank to treat everyone in while I went fallow. I'm running a FOWLR and moved my inverts over to a small temporary tank so I'm perfectly fine with dosing my DT and waiting an extended period of time to run Cuprisorb/etc to get it all out of the rock. In the process of slowly raising back my salinity to start Coppersafe I noticed that while there were still definitely spots on my fish, my butterflies especially, it never looked like outbreak levels of spots that I was accustomed to seeing when others would post about ich on their fish. The white spots on the butterflies would always be under 20 or so, and that would be on the high end. My wrasses are practically untouched, at most I would only see a spot or two every few days. My Banggai Cardinals have yet to show any outward signs of ich although I know it can also affect the gills. I also have a baby 1-1.25" Convict tang in the tank that I fully expected to be an ich magnet, but I have yet to see any spots on it.

Not to jinx myself or anything, but with all this I'd like to think that I've been able to manage the ich so far. I feed mysis/brine/TDO twice a day and soaked with selcon and every 1-2 weeks I do about a 20% water change where I siphon up some of the sand as well. I do have Coppersafe on hand in case anything starts to go wrong, but I've been hesitant with pushing the big red button especially since wrasses can be a little hit or miss with copper. Just looking for any input or other perspectives.

This may also be a consideration. @Jay Hemdal

I did go hypo before I left, but I'm pretty sure I have a hypo resistant strain. While I was gone the tank was at 1.009, verified by a family member, and I even elected to restart the 30-45 day period when I got back so that I could confirm the readings myself and had it down as low as 1.008 but the ich persisted.

I’ve never had ich survive a 1.009 specific gravity treatment for 30 days. There is reportedly a strain of ich or a related species from food fish culture in Taiwan that can reproduce at 1,008, but no indication that this has ever been seen in the pet trade.

Have seen this on ich management?

It's just so odd. I was ready to chalk it up to lymphocystis since my Threadfin had it before but it shows up occasionally on my wrasse as well. The only other thing I can think of is mucus plugs but to my knowledge something like that only occurs on tangs? Velvet would also be able to explain the spots but in this long of a time frame without any deaths? I can try to get pictures but it's a little hard to make out against the body of the fish.

Yeah I've looked at a bunch of posts regarding hypo, ich management, copper treatment LOL. Hydrogen Peroxide is an interesting option but I don't trust myself enough to get it right

Looked today and none of my fish are showing the white spots. And other than that I haven't observed any other telltale symptoms of things like velvet. No rapid breathing, clamped fins, etc. I may just go ahead and do the copper when I have the ability to monitor for an extended amount of time just for the peace of mind
