Just got this guy today, acclimated and into quarantine. Hes breathing hard and hugging the filter, anything I need to be worried about?

Attached is the best video I could get of the problem. The fish has developed a weird blackspot, I thought it was just gaining more spots. Now tubes/worms are coming out of it? They are white. Any idea what it is and what I can do to treat this immediately?

To note: The other clownfish I bought on the same day is 100% fine, eating, chosen an anemone to live in, swimming and breathing normally.

I don’t know much on diseases as I personally haven’t combatted many. It could potentially be an internal parasite though.
