Tube dwelling anemone made a new exit hole in itself. Should I be worried?

I've had my tuber for about 6 months. I think one of my conch snails exposed the middle of him a few days ago. I didn't think anything of it because it was eating the brine and other food floating around - as well as looking bright and healthy. He often goes all the way into his tube a couple times a day, but today, he came out the middle of him - leaving his normal exit area open.
I'm worried this new hole means he's hurt or the old exit area is infected. There's lingering dinos at the end, which the rest of the tank was eradicated of months ago, I left it because the easy methods of removing it hasn't worked and i didn't want to be too aggressive. Anyone experience this?

A hole was punched in his tube so he started using that hole instead of the previous one, correct? Just want to make sure I understand before I post anything.

I'm unsure! I just saw him poking out this new hole halfway through. Before today, the bottom half was barely "exposed" - still sand on it, but i could see its outline. Waiting for him to go back inside so I can see if there's damage, but its been about 10 hours and he's still out

I seen my feather dusters worms do similar things, sometimes a hermit will trim the tube and the duster just grows in a different direction, for most tube dwelling animals the tube being broken(as long as it's not at the base) is like you breaking a finger nail, it sucks but it's not gonna kill you. If he's still open I wouldn't worry too much just keep observing.
