That Didn't go as planned: What is something that did not go as planned for you

Sometimes we try something new with our reef tanks that seems like it will work out great, but it doesn't go as planned. Maybe it is adding a new fish that ends up not doing well in the tank, or maybe it is a complete overhaul of the tank, but something goes wrong. I am wondering what are those "didn't go as planned" moments for you
