Sailfin tang spots

my sailfin tang having small circle spots for past two weeks. But there is no change in character or rubbing against rocks. Can you clarify if it is an infection ...

It looks like there is a raised gray patch on its upper back. Are the fish's eyes cloudy also? they kind of look so. How long have you had this fish?
