My watchman goby died suddenly.

I have no idea what happened to him. This afternoon he was swimming from side to side, rolling in the sand, thrashing, and then laid on his side and finally died. I’ve had him for a month. Got him from my LFS where they QT all livestock. Yesterday, my levels were
Alk 149
pH 8.0
Phos 0.4
Ca 420
Mg 1417
Ammo 0.1
Nitrite 0.0
Nitrate 3

After testing, I did a 5 gallon water change (15 gallon tank). Shortly before he started being crazy, I dipped Zoas in revive and then iodine dip and returned them to the tank. Could that have affected him? He had no signs of illness as far as I know. My clowns are swimming around like normal. I’m so sad. Any idea what could have happened to him?

Its generally hard to tell once a fish has perished as to actual cause but I can tell you this fish is very thin and may be due to inadequate diet or even edema in which the fish was feeding off its liver and perished

I do worry about some of the dip chemicals getting into the tank - but since the clowns are ok, and I presume that the corals are also, I think we can rule that out.
