Is there something wrong with my Blue tang? HELP!

Hi guys,
Need a bit of help..

For the past week my blue tang has been looking stressed, her fins all out and flared constantly. Today as I went to feed the fish I noticed the front of her back fin looks like it’s disintegrating somehow… it just doesn’t seem right. It’s possible that it’s always been like this and I am only just noticing but wondered if this was a sign of anything? Is she getting sick? It doesn’t look like she’s been bitten to me but could that be a possibility? It just seems unusual.

Any advice or experience welcome please, she’s one of my favourites and really don’t want anything happening to her.

Appears to be very thin and has both fin rot and lymphocystis which both associated with poor water quality.
What are your test readings and age of tank ?
