I was on vacation for a week and my parents were feeding my fish. When we cam back my tusk was slowly moving around the top of the tank and just acting weird. On Sunday he ate four claims but that was the last time he ate. I tried a fresh water dip because I have had an issue with flukes six months ago and my lfs advised I start with that and treat the tank with Prazipro. This morning he look way worse and his fins are starting to rot away. I appreciate an help I can get. Tank is 300gal and he is the only sick fish. I did treat with prazipro half dose.

Images extremely dark. Really need under white light intensity. The one eye appears cloudy?
I suspect elevated ammonia and nitrate and a need for water change. As an all around safe cure would recommend Ruby Rally Pro which can be used safely in the tank AFTER a good water change. In lieu of clams, try scallops which is softer and one my tusk relishes. Prazi is a dewormer and wont help

Rally Pro doesn't work against flukes. If it was a water quality issue, I'd expect that other fish (and invertebrates!) would show symptoms. What other fish are in this tank?

With FW dips, the fish often look worse right afterwards, in fact, if the flukes are bad enough, the FW dip can kill a fish by dislodging all of the flukes and causing the fish to bleed out.

Better pics and video under white light may help....

Tank mates: epaulette shark, California round ray, baby bamboo shark (temporarily), purple tang, orange spotted Kole tang, yellow tang, marine betta, emperor angel, long nose hawk fish, green chrome, three anthias, two clowns, cleaner shrimp.

No other sick fish.

Some softy corals, no issue with them.

My tank is due for the weekly 50 gallon water change.

I will check the parameters asap. And get updated pictures.

I was able to get him in a bucket because he is just lying there. Not rapidly breathing and he moves quickly if you approach him with the net. I put him in a five gallon bucket with an air stone and new water. Ammonia and nitrates are not high in the tank but I’m going to do a water change because it is time for the normal. I appreciate the help with this.

Also, I forgot the parrot fish and scopus tang. Sorry was working and trying to save this guy.
Whoa - the tusk looks to be in a real bad way, hopefully it is still alive this morning.

I got a glimpse in one photo that made it seem like the tusk is really skinny, but I couldn’t tell for certain.

The cloudy skin and eyes and the fin damage can be a sign of flukes. However, with no other fish showing symptoms, that doesn’t line up well with that diagnosis. It could be a generalized bacterial infection, but those are rare. A fish dying from ich can look like this, but again, other fish would be showing symptoms.
