Hammer not doing well

Not really sure what’s going on with this hammer. Lost one that was the exact same size. Everything else in the tank seems to be doing well. The polyps look deflated, color is waning, flesh is developing brown spots.

The only change in environment is I moved it up about 8in it was fine there for probably a month then this started so I moved it to the sand bed to reduce light.

I did give it an iodine dip to see if that would help if it was bacterial or some type of hitchhiker. Dip was per instructions.

Are there any fish or inverts that could be bothering it? What about your flow? What about phosphate levels?

Only 2 clowns and a handfull of snails. It's hard to see from the photo but the flesh is starting to brown and the polyps are starting to lose their color.

Phosphate is .02

I had a hammer lose its color and almost look translucent before, I would check your nutrient levels. What lights are you using?

I had a hammer lose its color and almost look translucent before, I would check your nutrient levels. What lights are you using?
Using an ai prime 16. Yeah I have no clue. It may be worth it to do an ICP to see if there is some sort of deficiency occurring. Just as a note this is in a euphyllia dominant tank and everything else appears to be doing great. I do large weekly water changes with Tropic Marin Pro Reef.
