Can someone help my sick Angel? Laying sideways

There’s not one person in any fish store that can help… Our beautiful angelfish has been sideways… At the top of the tank… We gave a dose of antibiotic for five days and not helping… Does anybody really know how we can save this fish? 125 gallon tank.. water perfect, temp perfect…Tried epsom salt baths too… need help fast

This is a tough picture to work with - Can you provide a video?
Assure temp has not dropped below 75, ph is acidic and not high, epsom salt good. I need to determine further symptoms

The options IMHO are a swim bladder issue vs another condition that is causing air in the abdominal cavity/intestines. The picture as already mentioned is suboptimal. I would think the next step either way would be a freshwater antibiotic (broad spectrum) - in a hospital type tank (not the display). It wouldn't hurt, and may help. can you give more details? The size, age, other fish, and your parameters? Any other issues with the tank lately? Any other fish

He is worse today.. he was eating until yesterday and now he looks bloated… The antibiotics did not work… The Epsom salt did not work… The tank is 125 gallons… We had him in the hospital tank as well… Perfect water… Only a few small other fish… Nobody else is sick… he’s not gonna make it until I can find out what I can do…

Epsom salt wont work with saltwater as the fish is already in salt environment. Please add an updated pic for updated assessment. May be internal issue but appearance may offer clue

As mentioned, this being a saltwater forum, not many of us are well-versed in freshwater issues, but maybe @Jay Hemdal would know what's going on with your fish.

I used to keep these angels. Usually when this happens they are beyond help. Seen it with mine when I kept these guys. How hold is he? And can you post the video to youtube, I cant see the one you posted

Hi, we do try to help with freshwater fish issues here, but we need a clear picture and video of the fish, as well as other information - what other fish are in with this one, water quality, how long you’ve had this fish, what antibiotic did you use?
