Any idea what's going on with Albert the puffer?

42 gal corner wet/dry filter, skimmer (24/7), 18watt uv

tank established seven months ago, fish added three months ago.

Livestock - (1) dogface puffer
- (1) rusty angel
- (50 or so) small hermit crabs
Temp - 79.1F
Salinity - 1.026
PH - 8.4
Ammonia - 0
Nitrite - 0
Nitrate - 0
Phosphate - .25
DKH - 12
Calcium - 440
Magnesium - 1470

Puffer has thus far been healthy and eating well (frozen krill,bloodworms, silver sides and I'm certain he has snacked on a few of the hermys). About two days ago I noticed some swelling on his head, above his eyes. Still eating and fairly active (has always wedged himself in the rock). Today he is so puffy that his eyes appear to be sunken in. There are some photos attached. If anyone has any ideas as to what this is please share what you know or if have ever experienced this. Thank you so much.
