Look like tissue gone from an injury . Dont believe this is bacterial . Ive seen this with other angels and rabbitfish.

While it may Not be water quality related, assure to maintain good water quality and diet.
Flakes and pellets dont cut it but add meaty such as LRS nano frenzy
